Parents who will be accompanying their minors on-site fill out a registration form for each minor.
An minor unaccompanied by parents or legal guardians will not be admitted to an event unless two forms have been completed: the Minor's Consent to Participate and Hold Harmless Agreement, otherwise known as the Minor Waiver and the Medical Authorization Form. Each form must be notarized.
A set of forms must be completed for each unaccompanied child in the family.
