Calontir Award Recommendation Form Instructions

Welcome to the online version of the Calontir Award Recommendation Form. This form can be used in three ways;

Print out a blank Award Recommendation Form, the hand fill it out just like normal and deliver it to Their Majesties.
Partially filled out form - fill in all known info, then print out the page. Get any additional info or get others to add their names to the list, then deliver to Their Majesties.
Send a recommendation to Their Majesties or a designated representative directly by e-mail. Fill out all fields, then press Submit.
If you still manage to exceed this limit you will be notified when the rec is displayed. To correct just press the back button, you will see the same form with all of your fields still filled out, just change the comments section until everything fits or, if you must, send a second page. Placing "Page 2" or "Continued" in the comment field is recommended.
